Main Duties and Responsibilities
* Deliver specialist, evidence-based, patient-centred care for individuals with Parkinson's Disease
* Coordinate care across inpatient and outpatient settings
* Act as a resource for patients, carers, and healthcare professionals
* Support education, training, and service development
Job Responsibilities
* To provide specialist nursing expertise, facilitating evidence-based, patient-centred care and assessment for patients with Parkinson's disease
* To co-ordinate and monitor in-patients / out-patients throughout their assessment and treatment pathway
* To work with internal and external MDT and report or escalate patient wellbeing or deterioration in health
* To promote and support patients and carers to maintain quality of life balance
* To be involved in delivery of relevant programmes of education and development for all levels of staff and to act as a resource for patients and health care personnel
Person Specification
Essential
Education, Qualifications & Training
* Registered Nurse
* Evidence of CPD
* Teaching Qualification
Experience
* Significant recent experience managing patients with Parkinson's Disease and related conditions influencing the continued use of evidence-based practice
* Significant experience in working with patients with complex needs, in a hospital or community-based setting
Knowledge, Skills & Abilities
* Ability to practice as a proficient practitioner and undertake independent assessments
* Maintain effective working relationships with colleagues, patients and carers
* Experience of development and teaching other groups of staff
* Initiative and motivation to develop services
* Strong communication skills
Desirable
* Understanding of clinical governance and ability to assist in establishing systems
* Critical appraisal skills
* Ability to participate in audit and related research
