JOB SUMMARY
As a Social Worker within Adult Social Care, you will operate in a multidisciplinary setting, offering professional support to adults with physical or sensory impairments, learning disabilities, autism, and their carers. The focus will be on promoting independence, personalisation, and meeting social care outcomes as defined by the Department of Health’s Adult Social Care Outcomes Framework
JOB DESCRIPTION
The duties and responsibilities of the role will include:
Carrying out strength-based and proportionate assessments to develop personalised support plans.
Promoting long-term independence by identifying informal support networks and enabling community resources.
Assessing and arranging services such as equipment, reablement, housing adaptations, and assistive technology.
Managing a caseload, prioritising tasks effectively, and participating in duty and emergency response work.
Identifying and responding to safeguarding concerns in accordance with the Care Act 2014 and local procedures.
Working collaboratively across teams, including health and housing partners, to deliver effective support.
Keeping accurate records, contributing to legal proceedings when required, and supporting service improvement.
THE SUCCESSFUL CANDIDATE
The successful candidate will have the following skills and competencies:
Essential:
Recognised Social Work qualification (BA/MA/Postgraduate Diploma/Fast-track route)
Social Work England registration
Experience in multi-agency working and community-based practice
Strong understanding of the Care Act 2014, Mental Capacity Act, and relevant social care legislation
Skilled in safeguarding adults and applying “Making Safeguarding Personal” principles
Excellent assessment, communication, and case recording skills Desirable:
Post-qualifying training: Best Interest Assessor, Approved Mental Health Professional, or Practice Educator
