Your role and responsibilities As a Care Coordinator, your mission will be to support people in living well and independently at home. You’ll play a key part in transforming how care is delivered - by building strong, trusting relationships, streamlining services, and helping to reduce unnecessary hospital admissions. You’ll ensure care is delivered closer to where people live, making it more accessible, personalised, and effective. You will: Build trusted relationships with individuals and families to tailor support to their needs. Champion innovative projects and drive positive change. Work collaboratively with professionals across health, social care, and voluntary sectors. Navigate systems and services to ensure joined-up, responsive care.
