Specialist Nurse Looked After Children and Care Leavers

Job Summary
We are seeking an enthusiastic and innovative Band 6 Specialist Looked After Children and Care Leaver Nurse to deliver high-quality, compassionate care. The role involves leading statutory health assessments and addressing the health needs of children in care and care leavers.
Responsibilities
* Support the Named Nurse in delivering high-quality, effective, child-centered care.
* Lead statutory health assessments across settings (home, clinic, community).
* Manage own caseload, prioritise and coordinate care.
* Develop, implement and review individualized health care plans.
* Collaborate with multi-agency partners, attend meetings, and provide expert advice.
* Promote evidence-based practice and contribute to policy, procedures and clinical guidelines.
* Facilitate training for health professionals, foster carers and partner agencies.
* Support safeguarding, child protection and ensure children remain at centre of decision-making.
* Participate in quality improvement, audit and performance monitoring.
* Serve as a positive role model, support colleagues through supervision and shared learning.
* Engage in continuous professional development and maintain up-to-date knowledge.
Person Specifications
Essential:
* Registered Nurse (Child, Adult, Learning Disability or Mental Health) with the Nursing and Midwifery Council (NMC) and at least 2 years post registration.
* Experience working with looked-after children or vulnerable children/young people.
* Safeguarding and child protection experience.
* Community, nursing or public health experience.
* Multi-agency working experience.
Desirable:
* Health Visiting or School Nursing professional qualification.
* Evidence of post-graduate training in relevant topics.
* Evidence of Level 3 Safeguarding Children.
* Experience completing assessments, care planning and documentation.
* Ability to work independently and manage caseloads.
* Knowledge of statutory health assessments for children in care.
* Effective communication skills.

Band: 6 | Salary: £39,959 to £48,117 a year pro rata | Contract: Permanent | Working pattern: Part-time.
