The Role
We are seeking a Recovery Coordinator to support service delivery within Reconnected to Health at HMYOI Deerbolt. You will provide one-to-one support to service users, from brief interventions to structured treatment, including group work, tailored to individual needs. Adopting a holistic approach, you will ensure care plans address all aspects of wellbeing to optimize recovery outcomes.
* Manage a caseload, developing, implementing, and reviewing recovery plans in collaboration with the service team, including family and significant others where appropriate.
* Conduct triage and comprehensive assessments with service users.
* Work with partner agencies on shared assessments, key working, and interventions to support progression from engagement through treatment to recovery.
* Facilitate access to treatment and community resources, supporting service users and families within their local communities. Lead case management activities, including risk assessments, risk management, safeguarding, and documentation using online systems.
