Overview
Our Clinical Decision Making (CDM) team is based within the Single Point of Access (SPoA) for Community Services in Wye Valley NHS Trust. The team is growing to support the development of community services.
We are looking for enthusiastic and experienced nurses who have a robust understanding of community nursing and are able to provide clinical advice and support for urgent care and admission avoidance.
Responsibilities
In this post you will support the clinical triage and streaming of referrals in our Community Referral Hub. Referrals are taken directly from professionals, allowing clinicians to undertake remote assessments over the phone as required. In collaboration with patients, carers and professionals you will decide which service is best placed to meet their needs and avoid a crisis or admission.
You will be based in a truly integrated, multi‑organisational hub that manages referrals and patient queries for the District Nurses, Urgent Response Service, Virtual Wards, Virtual GP, IVOPAT, Therapies (OT/PT) and Falls Support. The main duties include: triaging direct telephone referrals, offering clinical advice and coordinating support for patients who need urgent or same‑day care. Admission avoidance is a key focus.
The role also involves liaising directly with patients and referrers, supporting Administrators and Call Handlers, and ensuring efficient, effective service delivery in the hub. You will work proactively and approachably, supporting junior staff and ensuring the day‑to‑day running of the hub in support of clinical leads and administration managers.
