Job Purpose
* To deliver high-quality occupational therapy assessments and interventions within social services
* To support adults in maximising independence, safety, and wellbeing within the community
* To work collaboratively with multidisciplinary professionals to deliver person-centred care
Key Responsibilities
* Complete comprehensive occupational therapy assessments for service users
* Deliver targeted interventions to support daily living activities and promote independence
* Develop, implement, and review personalised care and rehabilitation plans
* Work closely with the Social Services Team to ensure coordinated and effective care delivery
* Liaise with healthcare professionals, families, and external agencies to ensure holistic support
* Maintain accurate, timely, and compliant records and documentation
Essential Requirements
* HCPC registration as an Occupational Therapist
* Experience working within social services or community-based settings
* Strong assessment, intervention, and care-planning skills
* Excellent communication and organisational abilities
* Ability to work independently while contributing effectively to a wider team
