Job Details
* Role: Weekend Supported Housing Worker
* Salary: £8,075 per year (full-time equivalent £25,235) | £12.94 per hour
* Hours: 12 hours per week
* Contract: Permanent
* Location: Somerset East & Somerset (mainly Frome & Shepton Mallet)
* Additional Requirements: Valid driver’s license, access to a car for business purposes, participation in an on-call rota (£100/week when on-call), working mainly on weekends (Saturday and Sunday)
The Role
As a Support Worker, you will provide person-centered support and housing management to individuals with high support needs, many of whom have experienced rough sleeping. You will work across four dispersed units, building strong relationships with residents to foster engagement, reduce barriers, and support tenancy maintenance and independence.
You will collaborate closely with weekday Support Workers to ensure consistent support, assisting approximately four clients on their journey toward stability and independence.
Responsibilities
* Progress and track referrals and outcomes for clients
* Conduct holistic assessments and develop personalized support plans
* Provide advice, information, and enable clients to access appropriate services
* Encourage client involvement and ensure their voice is heard
* Maintain accurate and up-to-date client records
* Perform additional duties aligned with key responsibilities as needed
Qualifications and Requirements
* Valid UK driving license and access to own vehicle, with business insurance for work-related travel
* Experience working with vulnerable adults and/or individuals with complex needs and challenging behaviors
