We have an exciting opportunity for an enthusiastic and proactive pharmacist to join our supportive team delivering innovative pharmacy services to the community.

No two days will be the same; you will have patient contact; you will work as part of a multi-disciplinary team; you will problem solve and you will have the opportunity to shape the pharmacy service you deliver.

Prospective candidates should be efficient, enthusiastic and confident to work autonomously within a supportive team and their sphere of competence. They should have excellent communication skills as the job will involve liaising with social care, clinical and community teams as well as patients and their carers.

You will have a desire to improve patient outcomes; ensure our medicines governance is compliant with CQC standards, the will to learn and develop and the opportunity to influence the future of pharmacy services within the scope of the new NHS 10 year plan.

An independent non-medical prescribing qualification would be ideal. If however you are not currently a prescriber, you will be supported to undertake this.

You will need to be flexible in your approach to work and able to travel as the role involves hybrid working.

Experience in community services is not essential but a strong clinical background would be helpful.

This post is 30 hours a week.


Main duties of the job

You will be working as part of a multidisciplinary team supporting our four primary care home teams delivering patient centered care. You will also have the opportunity to provide clinical input into our two community hospital in-patient wards (Beccles Hospital and Carlton Court) as well as the specialist palliative care unit delivered by St Elizabeth's Hospice.

The Clinical Pharmacy Service must be safe, evidence based, innovative, cost-effective and high quality.

You will be responsible for ensuring that there is compliance with medicines legislation; national and local guidance and to manage risks associated with medicines use. Audits, improvement projects and resulting action plans in line with the Pharmacy yearly plan will also be completed.

To ensure appropriate governance is in place regarding medicines, including leading on the response to any drug safety alerts or changes in clinical guidance, responding to incidents and escalate as appropriate.

To provide professional expertise, advice, support and training on medicines optimisation policies to health professionals.

To organise and participate in regular training and feedback sessions as appropriate to a variety of clinicians and support staff.

Mentor the Pharmacy Technicians and other pharmacy support staff and respond to their queries in a supportive and timely manner.

To deputise where appropriate (and agreed) for the Operational Lead or Head of Pharmacy.

Job responsibilities

To develop and provide a high-quality Clinical Pharmacy Service to ECCH Primary Care Home Teams, including medicines reviews, polypharmacy, medicines reconciliation and drug monitoring to ensure safe and cost-effective use of evidence-based medicine in line with local policies and national guidance.

To support the provision of clinical pharmacy services to the community hospital ward and specialist palliative care beds. Participate in ward rounds and MDTs where appropriate.

To provide professional expertise in relation to drugs or medicines to PCH staff; patients and carers and others including medical practitioners.

To support and implement efficient medicines optimisation initiatives to ensure the safe and secure handling of medicines; reduce drug waste and manage drug expenditure.

To prescribe within sphere of competence and where appropriate the necessary medicines to meet the needs of the patient.

To participate in MDTs and PCH huddles where appropriate.

To work closely with pharmacy technicians to enable patients in our care to be empowered to manage their medicines.

To facilitate the medicines element of discharge in a safe and effective manner.

To participate in and undertake audits as agreed, including collecting and recording data for KPIs.

To support the wider pharmacy and ECCH teams in delivering safe patient-centered care through implementation of the Pharmacy and Medicines Optimisation strategy including providing professional advice; training and education; developing and reviewing relevant policies and SOPs.

To lead and manage pharmacy support staff where required.

Person Specification


Qualifications

* Masters degree in pharmacy or equivalent
* Postgraduate clinical qualification and/or significant relevant clinical experience to demonstrate similar level of knowledge
* Registered with the General Pharmaceutical Council
* Management or Leadership qualification
* Holding or working towards a non-medical prescribing qualification


Experience

* Significant post-registration experience in acute hospital, community health services or community pharmacy
* Ability to analyse, interpret and evaluate specialist medicines-related information or data, using clinical and professional judgement, including reports from pharmacy computer systems, clinical intervention reports, data from clinical trials, and changes in medicines legislation and policy.
* Experience in planning, developing, and delivering pharmacy services
* Responsible and accountable for own professional actions, working independently within professional and organizational boundaries, and supervising others within their area of responsibility
* Experience working with end-of-life patients, gained through hospice or specialist center experience
* Technical knowledge and skills for preparing injections and intravenous infusions
* Teaching and training experience for healthcare professionals, students, and undergraduates
* Experience in various environments such as community, hospital, and primary care
* Experience of service redesign
* Experience using SystmOne

* Ability to communicate complex, sensitive medicines-related information effectively to patients, carers, and healthcare professionals, including in sensitive situations such as cancer treatment or terminal illness
