Beyond Vision Transitions is a high quality semi-independent/supported living service, providing 24hr support for vulnerable looked after people between the ages of 17 +, who have left care, are autistic, are recovering from an eating disorder or are struggling with their mental health.
We work in partnership with local authorities to develop tailored packages of 'Support' to meet the individual needs of each client.

We want all people to have an opportunity to explore themselves and develop a greater understanding of their behaviours and abilities to break the cycle of negative behaviours, to transform their lives and reach their potential.
We are looking for support workers who will work in a provision with up to 3 people. They will answer to and work closely with the manager and be expected to provide a bespoke package of support for each young person based on their needs and goals. They will work alongside social services and other external agencies to facilitate support.

* Someone with a robust understanding of safeguarding and promoting individual rights
Role & Responsibilities:
* Ensure the health, safety and well-being of each client and that safeguarding procedures are followed throughout their care.
* Maintain the confidentiality of people and report any concerns regarding the person's welfare to your line manager.
* Attend meetings ensuring all relevant paperwork is completed.
* Mandatory participation in training sessions as and when required.
* To maintain the cleanliness and hygiene of the Home, reporting all repair issues and ensuring the environment is always welcoming.
* Promote safe, consistent and understandable boundaries with all people in conjunction with a ' Person's Agreement', Support Plan and House Rules.
* Prepare detailed high-quality monthly progress reports regarding young client's progress which is submitted to the placing authority within deadlines.
* Work in partnership with social workers and other agencies and to encourage and support the people to attend meetings with other professionals.
* Assist the clients to engage with everyday living and develop skills where necessary by supporting their identified needs.
* Provide support to all clients with budgeting money, grocery shopping and administer a weekly subsistence allowance where necessary.
* Encourage and support clients in accessing education, employment or training as well as supporting them with the application process.
* Maintain accurate records, files, log keeping and all other required paperwork and administrative requirements.
