Job Summary

The Intensive Support Team (IST) offers a rapid, time-limited, community-based assessment and treatment service for people with learning disabilities in Buckinghamshire who challenge services and have additional mental health needs. IST provides an alternative to hospital admission where appropriate, aiming to minimise disruption to service users and their carers.


Main duties

* The team delivers care for individuals with learning disabilities, mental health and/or challenging behaviour needs in their own homes.
* Assist with the development and implementation of care plans and assessments in partnership with individuals and their families/carers.
* Participate in a multidisciplinary team, contributing to Positive Behavioural Support plans.
* Travel between sites as required; must hold a current UK driving licence and have reliable transportation.


Responsibilities

* Work with an allocated caseload under the supervision of a qualified practitioner.
* Respect and acknowledge the physical, psychological, emotional, and environmental needs of service users and carers during contacts.
* Plan and lead group and individual interventions in conjunction with the case holder.
* Carry out delegated tasks without direct supervision in a variety of settings, including learning disability and mental health inpatient services.
* Attend and contribute to meetings as agreed with case holders.
* Support the assessment and treatment process to enhance service users’ physical and psychological well‑being, including transport to and from services where appropriate.
* Keep accurate records of tasks undertaken, observations, and events concerning service users and report to the relevant team members.
* Prepare reports on individual service users when requested.
* Maintain and promote good communication and working relationships within the team and with other agencies.
* Develop knowledge and understanding of specialist needs and the approaches used in care plans.
* Participate in all mandatory training and professional development opportunities to maintain skills and knowledge.
* Report and document any changes in a service user’s health or behaviour to a senior member of staff.
* Report incidents to a senior member of staff.
* Participate in regular supervision and the annual appraisal process.
* Induct new staff members, students, and trainees.


Qualifications

* Essential: Evidence of academic achievement to at least GCSE level or equivalent in English, Maths, and a science subject.
* Desirable: Relevant care qualification.


Experience

* Experience working with people who have learning disabilities and may have mental health needs and/or challenging behaviour.


Skills and Knowledge

* Work under the direction and indirect supervision of qualified practitioners.
* Deliver the Band 4 role within an agreed care plan.
* Excellent communication skills, including the use of various communication methods.
* Awareness of risk assessment and Positive Behavioural Support frameworks.
