We’re thrilled to announce an opening for a Frailty Pharmacist(s) (B7) in our newly established Frailty Unit at NHS Borders.
In this pivotal role, you will work alongside a multidisciplinary team (including the wider Frailty team) to optimise medication management, enhance patient care, and support our commitment to addressing the complexities of frailty in our community.
Key Responsibilities
* Conduct comprehensive medication reviews for frail patients.
* Collaborate closely with healthcare professionals to develop individualised care plans.
* Provide expert pharmaceutical advice to enhance patient safety and medication efficacy.
* Educate patients and caregivers about their medications and health conditions.

* A passionate pharmacist(s) with experience in geriatric care or a keen interest in frailty management.
* Strong communication skills and a commitment to teamwork.
