NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. The role:

The Community Rehabilitation and Reablement Service provides specialist rehabilitation across East Renfrewshire HSCP. The multi‑disciplinary team consists of physiotherapy, occupational therapy, dietetics, speech and language, nursing, assistant practitioner and rehabilitation support worker intervention. The Service operates over two geographic teams in Eastwood and Barrhead localities and provides supported discharge from Acute settings and coordinated assessment and treatment within an individual’s own home (or other appropriate community or workplace setting) to maximise health gain and independence.

Staff work as part of a flexible and responsive team which aims to provide a single point of access to assessment of need followed by time‑limited and appropriate rehabilitation. Close collaboration exists with service users, carers, primary and secondary healthcare, social work services and the voluntary sector.

Applicants should have a degree or diploma in dietetics and be registered with the Health Professions Council. Significant post graduate experience of which substantial years should be at senior level. Experience in Adult Rehabilitation, Neurology, Orthopaedics, Medicine or Care of the Elderly Specialities is beneficial.

A current full driving licence is essential for this role.


Duration, Location, and Working Pattern:

* This is a permanent part‑time post of 28.8 hours per week.
* Location‑based in Eastwood Health and Care Centre.
* Shift pattern for this post is 4 out of 7 days.
* Please note the salary is Pro Rata for part time hours.


Key responsibilities:

1. Undertake a comprehensive assessment of patients (including screening and multi‑professional baseline assessments) with a complex presentation using investigative skills, analysing clinical and non clinical information (e.g. the interpretation and analysis of appropriate tests, biochemical data, anthropometric measurement and nutritional screening tools as well as social and environmental information) and to provide a functional diagnosis for individual patients to determine their need for Dietetic intervention.
2. Provide specialist interventions based on assessment, diagnosis, treatment and associated management of the nutrition and dietetic needs of the client. Monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention.
3. Following assessment, act autonomously to assess, plan, implement and evaluate Dietetic interventions for a range of patients. This will include patients who have highly complex presentations and those with multi‑factorial health and social needs, requiring multi‑service/multi‑agency input. Incorporate risk assessment to maximise functional independence and rehabilitation potential within a specialist area.
4. Provide specialist input to the identification and management of disease‑related malnutrition, e.g.:
o Diabetes
o Chronic neurological patients
o Obesity
o GI disorders
Understanding the significance in its role in therapy outcomes.
5. Manage Dietetic intervention of patients presenting with dysphagia. This multifactorial approach includes positioning, texture modification of diets and nutritional supplementation. Assessment and management of patients requiring enteral feeding i.e. nasogastric, gastrostomy and jejunostomy feeding and their onward referral from the service.
6. Undertake a case coordination role taking responsibility for coordinating the interdisciplinary goal planning and discharge planning process. Liaise and work collaboratively with other agencies e.g. initiating referrals, sharing of information, recommending appropriate service delivery, attendance at case conferences and discharge planning. Balance the needs of a clinical caseload alongside case coordination duties in the management of service waiting times.
