We are seeking an enthusiastic, motivated, and forward-thinking Occupational Therapist to join our Mental Health Hospital Discharge Countywide team on a fixed-term 12-month contract. You will work with both working age adults (18-70) and older adults (70+), supported by a caring team dedicated to both your well-being and community service.

Main duties of the job

You will facilitate effective and timely hospital discharges across East Sussex, covering all acute hospitals for patients admitted for mental health treatment. This is a new initiative; you will work as part of a remote partnership team with Sussex Partnership Foundation Trust (SPFT) and the voluntary sector to develop safe, creative discharge plans centered on the individual.

You will collaborate closely with various stakeholders to ensure the best discharge outcomes.

Job responsibilities

1. Undertake professional assessments and make management recommendations regarding disability and rehabilitation.
2. Advise and influence stakeholders on environmental issues related to disability and access.
3. Apply specialist knowledge in advice, counselling, support, training, equipment provision, and remedial treatments.
4. Guide individuals, families, and colleagues on managing disabilities.
5. Provide information and advice about alternative services and support sources.
6. Collaborate with colleagues, professionals, and agencies for client benefit.
7. Organize group work for individuals, families, and carers.
8. Assist in assessments for residential, day, or family-based care admissions.
9. Support supervision and delegation to staff supporting the OT service.
10. Participate in staff and student training initiatives.
11. Keep the Senior Occupational Therapist informed on local service development matters.
12. Complete administrative tasks to maintain service quality.
13. Maintain professional standards.
14. Support case management, documentation, and present reports as needed.


Person Specification


Qualifications

* Occupational Therapy qualification and registration with the relevant regulatory body.


Experience

* Effective communication skills, including report writing and liaising with the public and agencies.
* Understanding of assessment principles and application.
* Ability to plan, organize, and meet deadlines.
* Excellent spoken English and advice-giving skills.
* Knowledge of legislation affecting services for older people and adults with disabilities.
* Understanding of disability causes and effects.
* Familiarity with key OT roles and experience working within a team and under pressure.
* Experience in environmental redesign, preferably in physical disability settings.
* Ability to work independently and responsibly.
* Commitment to equality and community development.
* Willingness to meet travel needs.
* Experience in designing adaptations and understanding grants procedures.
* In-depth knowledge of disabilities such as MS, spinal injuries, CVAs, MND, cerebral palsy, and spina bifida.
* Experience working with people with disabilities in statutory or voluntary sectors.
