Role Overview
The post holder effectively participates in the provision of clinical and operational pharmaceutical services to Chelsea and Westminster NHS Foundation Trust (West Middlesex site) under appropriate supervision for grade and experience, rotating through various specialist areas on a 4‑month basis. They also provide an out‑of‑hours weekend and on‑call service on a rostered basis.

Key Responsibilities
* Participate in 4‑month rotations as per the rota and complete competencies in each rotation.
* Provide out‑of‑hours weekend and on‑call service on a rostered basis.
* Patient Services responsibilities:
o Provide legally required supervision and validation aspects of the dispensing process.
o Dispense medicines for inpatients and outpatients (including clinical trials and controlled drugs).
o Accuracy checking of pharmacy technician/assistant/trainee pharmacist/trainee pharmacy technician dispensing.
o Counsel outpatients to local, national and legal standards.
o Support management of stock control, dispensing workload and staff and contribute to quality service and clinical governance including reporting near misses and errors.
o Contribute to training of new and junior pharmacy staff and students on placement.
o Maintain high quality patient data input and ensure excellent labelling standards.
o Contribute to the update and production of Standard Operating Procedures.
o Undertake specific projects in accordance with objectives set.
* Clinical Pharmacy Services responsibilities:
o Rotation through specific clinical areas within the hospital.
o Provide ward/clinical pharmacy services to the wards of the Trust to defined local and regional standards.
o Identify pharmaceutical problems for patients and apply therapeutic knowledge.
o Provide clinical pharmacy responsibility for all patients on the assigned ward.
o Ensure accurate medication history is taken, documented and reconciled within the appropriate timeframe.
o Ensure discharge prescriptions are safe, accurate, legal and clinically appropriate; recommend changes to prescribers.
o Advise other professional staff on medicines management and optimisation issues.
o Participate in clinical intervention systems and quality monitoring initiatives.
o Implement local formulary recommendations / therapeutic guidelines and support Divisional and Specialist pharmacists.
o Implement risk management systems and report incidents.
o Participate in clinical training schemes, attend and complete coursework and assessments.
o Review all medications prescribed on the electronic prescribing system for each patient on the assigned ward.
o Prioritise and resolve pharmaceutical problems, liaising with other healthcare professionals.
o Supply prescribed medication to patients on allocated wards in a timely manner.
o Review stock holding of medication on the ward with nursing staff.
o Review prescriptions for accuracy and legality, identify potential problems.
o Ensure effective supply of medicines suitable for inpatient use and appropriate labelling for discharge.
o Monitor patients for potential adverse effects from their medicines.
o Ensure effective treatment outcomes with medicines.
o Ensure patients understand the purpose of their medicines and address practical issues.
o Support planning of patient discharge with respect to medications, including transcription of dispensing requests.
o Demonstrate professional accountability to patients.
o Participate in developing medicines management systems and auditing outcomes.
o Ensure all medicines (including controlled drugs) are stored according to requirements and conduct monthly stock checks.
o Undertake training in specialist areas as service and development needs arise.
* Independent Prescribing (where applicable)
o Work within scope of practice as a generalist prescriber and utilise prescribing principles in accordance with the Trust non‑medical prescriber policy.
* Technical and Cancer Services responsibilities:
o Participation and accreditation in all aspects of technical and cancer services, including final checking and releasing of aseptically prepared systemic anticancer therapy.
o Clinical screening accreditation for oral and parenteral chemotherapy.
o Awareness of GMP and other procedures, policies, guidelines and risk management principles.
o Support provision of clinical pharmacy services to haematology and oncology.
* Additional/Other responsibilities:
o Deputise for relevant senior staff.
o Participate in weekend/public rota, on‑call rota and late duty rota.
o Participate in performance review and appraisal and identify own learning and development needs.
o Participate in audit and project work in line with service development.
o Assist in training and supervision of pharmacy technicians/trainee pharmacy technicians/assistants/trainee pharmacists and pharmacy students on placement.
o Promote safe, economic and effective use of medicines.
o Maintain a written Continuing Professional Development record.
o Attend and participate in departmental meetings and quality initiatives.
o Follow legal, ethical, professional and employer's codes of conduct.
o Demonstrate awareness of and commitment to the Trust's Clinical Governance Agenda.
o Perform any other duties requested by the Chief Pharmacist or their Deputy.
