Reablement Assistant Practitioner
Permanent contract, 37 hours per week, £32,061 to £33,699 per annum (Grade G) – based in Greyfriars House, Great Yarmouth.
Overview
Work in the community with a well‑trained team of Occupational Therapists and support workers to create person‑centred personal plans that help people set and achieve goals following illness or hospital admission. You will visit clients in their own homes across East Norfolk and support them to regain independence.
Responsibilities
* Participate in multidisciplinary assessment teams alongside Occupational Therapists and other professionals.
* Visit clients in their homes across East Norfolk.
* Assess and review daily living needs, developing personalised plans to restore skills and confidence.
* Complete Care Act Assessments when ongoing services are required.
* Plan and prioritise work using judgment and initiative, and communicate clearly with clients and the public.
* Work Monday to Friday; weekend work may be reintroduced as required.
Qualifications
* Experience working with people who need support in a home environment.
* Clear communication skills with clients, the public and professionals.
* Knowledge of current health and social care policy, practice and legislation.
* Ability to exercise judgement and plan work independently.
