Overview
This role is a Band 5 Community Nurse, part-time 25 hours per week (18:30–23:30, over 5 evenings) as part of the evening team, or 2 posts at 15 and 10 hours. The team is countywide and centrally based in Droitwich with satellite bases in Evesham, Bromsgrove and Kidderminster. This post is primarily based in Droitwich, covering South Worcestershire. You will visit patients in their homes to help them remain at home, prevent hospital admissions and support early discharge from hospital. The role is paid for unsocial hours with enhancements.

Key responsibilities
As a community nurse in the Evening Team, you will contribute to a 24-hour admission avoidance service across Worcestershire in urban and rural settings. Responsibilities include:
* Taking direct referrals from patients, 111, the Ambulance Service and discharge hospitals
* Independently managing a caseload, with initial assessments often performed in the community
* Providing care for diverse needs (catheters, wound care, palliative care, diabetes management, medication administration, rehabilitation following discharge)
* Starting visits from home as geography requires and working with a handheld tablet for triage
* Collaborating with West Midlands Ambulance Service and 111 to respond to urgent care needs and prevent hospital admissions
There is a comprehensive induction with peer support before working independently.

Salary: £31,049 to £37,796 per year (pro rata) under Agenda for Change, Band 5. Permanent contract. Part-time, flexible working. Location: Droitwich Medical Centre, Ombersley Street East, Droitwich, Worcestershire, WR9 8RD.
Qualifications and experience
Essential
* Registered Nurse (RN1)
* Ability to travel as required
* Experience gained during pre-registration training
* Ability to meet the travel requirements of the role
* Current UK professional registration
Desirable
* Accredited courses in palliative care, tissue viability, diabetes
* Mentor award
* 2 years post-registration experience (community nursing preferred)
* Experience in management of caseloads
