Job Description
What you’ll do?
Every day as a Senior Care Assistant is different, but your impact is always the same - life-changing.
You will:
4. Support prompt, safe hospital discharges back home (within 24 hours of referral)
5. Empower clients to regain independence, confidence and skills through positive, safe risk-taking
6. Build trusting relationships and provide emotional encouragement every day
7. Adapt home environments to reduce risks and suit individual needs
8. Help clients achieve goals set by social workers with minimal ongoing support
9. Establish motivating daily routines that promote great self-care
10. Assess capabilities (physical & cognitive) over 14 days, gather evidence, and plan the right-sized care package
11. Refer clients to supportive tech (e.g. reminder clocks, medication dispensers, door sensors, GPS watches)
12. Work closely with families to create person-centred care plans that everyone loves
13. Attend multi-agency meetings, provide progress updates, and collaborate to increase independence
14. Write tailored care plans, complete risk assessments, and deliver thorough handovers to long-term teams
15. Connect clients with community groups and services to boost social skills and integration
16. Take charge of your own professional development – source relevant training and keep skills refreshed
Qualifications

17. Proven experience in domiciliary/home care (essential)
18. A genuine person-centred approach – always putting the individual first
19. Strong communication skills – excellent verbal, non-verbal, and warm demeanour to connect with clients, families, and teams
20. Compassion, empathy, and a real passion for supporting others
21. Proactive mindset with creative initiative and the ability to think outside the box
22. Solid problem-solving skills and a holistic view of care needs
23. Knowledge of positive risk-taking and how it safely promotes independence and confidence
24. Clear understanding of the care sector, including promoting independence and managing logistics in home settings
25. Great team player – building positive relationships with clients, keyworkers, families, and colleagues
26. Flexible, reliable, and punctual – able to adapt to changing needs and keep things on track
