Key Responsibilities:
Deliver prompt, high-quality nursing care as part of a Community Rapid Response service.
Perform thorough clinical assessments and create patient-specific care plans in partnership with multi-disciplinary teams.
Provide guidance and emotional support to patients and families, promoting self-management and understanding of care plans.
Coordinate with GPs, hospital teams, therapists, and social care professionals to ensure holistic and seamless patient care.
Keep meticulous patient records, ensuring all interventions and observations are accurately documented.
