The Community Rehabilitation Service focus is to provide rehabilitation support to patients within their own communities to achieve what is important to them. We enable our patients to regain lost skills, increase their levels of independence and wellbeing and to empower and equip them to make lasting changes for their future. We give them support and guidance to help them to achieve their goals and encourage them to contribute to decisions about their treatment.
Main duties of the job
Job Purpose: To provide high quality patient-centered care and specialist rehabilitation to adults referred to the Rehabilitation Service. Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded environments. To provide specialist advice regarding the rehabilitation management of patients to families, carers and other members of the MDT. Provide support, guidance and training to Band 5 Therapists, Apprentices, Rehabilitation Assistants and Students. Responsibility for a caseload of complex patients within both the community and the community hospital bedded environments. Act as Clinical Supervisor and undertake Managerial Supervision including Appraisal for Band 5 Physiotherapists and Rehabilitation Assistants. Act as Team Lead in the Rehabilitation Managers absence, including supporting staff and attending any meetings delegated by the manager.
