Job Title: Social Worker - Preparing for Adulthood - Learning Disabilities & Autism Location: Rochdale Pay Rate: £35 per hour Liquid Personnel is recruiting an Adult Social Worker to join its client’s Preparing for Adulthood - Learning Disabilities & Autism Team, based in Rochdale. What will your responsibilities be? Conducting Care Act assessments: Evaluate young people (usually 17) to determine eligibility for adult social care services. Coordinating transition planning: Work with schools, families, and professionals to ensure smooth movement from children’s to adult services. Supporting independence and community inclusion: Help individuals develop daily living skills, access housing options, and engage in employment or training pathways.
