Social Worker – Care Act Assessments Pertemps is recruiting an experienced Social Worker to join an Adult Social Care team in Aylesbury, focusing on high‑quality Care Act assessments and person‑centred planning. This is a key role supporting adults to maintain independence, safety and wellbeing.
