Advanced Nurse Practitioner, Band 7
An exciting opportunity has arisen to join our Advanced Nurse Practitioner team within Perth & Kinross Health & Social Care Partnership as part of our Medicine for the elderly / Locality Integrated Care Team.
The Advanced Nurse Practitioner role within offers individuals the opportunity to develop their career in advanced practice; improving patient outcomes and pathways of care for frail, older adults in the community.
The post holder will have experience of working at Advanced level within a clinical setting, demonstrating excellent leadership and communication with the ability to work autonomously across the four pillars of advanced practice. Applicants will have the ability to provide clinical leadership through comprehensive patient history taking, advanced clinical assessment, differential diagnosis, problem solving and treatment planning, incorporating non-medical prescribing.
The post holder will play a key role in the clinical leadership and development of advanced nursing practice within the community, delivering high quality evidence based and person-centred care to frail, elderly patients within the community.
You will pro-actively manage frail elderly adults and you will be expected to act as a clinical leader working in close collaboration with the Integrated Care Teams, Community Hospitals and Medicine for the Elderly Consultants.
This role will be fully supported by the Lead Advanced Nurse Practitioners and candidates will be supported to develop by working collaboratively with all members of the community multidisciplinary teams.
Evidence of continual professional development within an appropriate clinical setting, the proven ability to work toward Masters Level learning in Advanced Nursing Practice and a qualification as an Independent Nurse Prescriber is essential. The applicant will also need to demonstrate extensive and varied post registration experience and be able to travel across Perth and Kinross to undertake the role. Successful candidates will play a key role in enhancing person centred care and experiences, assuring clinical governance and supporting the Lead Advanced Nurse Practitioners, ultimately improving outcomes for frail, elderly patients in the community.
You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the UK NMC Register
