JOB TITLE: Support Worker
REPORTING TO: Placement Manager / Registered Manager
JOB PURPOSE:
To work within a team to provide support services for young people aged with learning disabilities, mental health issues and/or behaviours that may challenge; which aims to maximise the potential of individuals in line with ordinary life principles.
SKILLS / ABILITIES
Required:
Ø Proven ability to work on own initiative and work as part of a team
Ø Knowledge and understanding of issues relating to people with behaviour that challenges
Ø A commitment to a service which provides support to vulnerable individuals, and which is respectful and according to the individuals' wishes
Ø Good communication skills (written and verbal). Ability to communicate effectively with service users, colleagues, managers and other persons with a legitimate interest in the buisness

EXPERIENCE

MAIN RESPONSIBILITIES
1. Work within the philosophies of the organisation to minimise the effects of Service users behaviours, demonstrating an understanding and commitment to the rights of people with past trauma.
2. Facilitate and support service users in decision making and promoting choice and involvement giving full regard to initiatives aimed at promoting service user empowerment.
3. Ensure Service users individual needs are met through effective pathway planning and review. Review and monitor support plans
4. Support service users participation in social, leisure, educational and employment opportunities within the local community in line with ordinary life principles.
5. Act as a Key worker to an individual service user, taking on responsibility for specified areas of support. Ensure all medical needs are met and supporting the service user to advocate his/her own needs and wishes.
6. Consistently apply non aversive strategies developed in response to challenging behaviours and contribute to the review and development of any such strategies.
7. Encourage and enable service users to maintain acceptable levels of personal and environmental hygiene
8. Interact and participate with the service users in the development of self-help skills, supporting service users to reach their potential for independence.
9. Liaise with other agencies/professionals as necessary in consultation with senior staff members
10. Record information about service users and ensure communication with all staff members
11. Ensure that medical emergencies are dealt with appropriately and adhere to the company's medication policy at all times.
12. Ensure that service user's personal property and clothing is maintained to a high standard and reports any requirements to senior staff members.
13. Provide care and support as identified in support reviews and support plans, as directed by senior staff members.
14. Take responsibility for running a shift as delegated to do so.
15. Practice maximum integrity in all dealings with service users personal and financial affairs, and avoid abuse of the privileged relationship that exists with service users
16. Participate in staff and service user meetings as and when required
17. Maintain professional knowledge and competence.
18. Attend mandatory training days/courses, on or off site as and when required.
19. Participate in relevant NVQ training to achieve required qualifications
20. Report immediately to the placement manager or registered Service Manager any illness of an infectious nature or accident/injury incurred by a service user, colleague, self or another.
21. Understand and ensure the implementation of, the organisations health and safety policy and emergency and fire procedures.
22. Report to the placement manager or landlord any faulty appliances, damaged furniture, equipment or any potential hazard where appropriate
23. Promote safe working practice within the organisation
24. Assist in maintaining financial records in line with Company policies and procedures.
25. Carry out administrative tasks as and when required.
26. Share in the completion of domestic tasks to assist in the overall running of your place of work. Share responsibility for shopping and preparing meals, and ensuring environmental hygiene is met through the completion of required cleaning tasks. These should not be done in isolation but should involve service users.
27. Make visitors feel welcome. Provide refreshments/assistance as and when required.

29. Ensure that all information of confidential nature gained in the course of duty is not divulged to third parties.
30. Work in a team, contributing to effective team working and participating in reviews of service delivery standards.
31. Notify the placement manager as soon as possible of your inability to report for duty, and also on your return to work from all periods of absence.
32. Ensure the security of your place of work is maintained at all times.
33. Adhere to all company policies and procedures within defined timescales.
34. Ensure all equipment is clean and well maintained.
35. Carry out any other tasks that may be reasonably assigned to you.
