University Hospitals Tees is seeking a candidate for the Pharmacy Virtual Ward Team. This role focuses on promoting the safe, rational use of medications for patients managed within the Virtual Ward.
The post holder will undertake medicines reconciliation, provide counselling, and support the supply of medicines, ensuring patient safety. Training opportunities in leadership and quality improvement will be provided.
