District Nurse Clinical Lead (Band 7) – Community Nursing Services

Permanent – 37.5 hours per week. Vacancy: Seaford Locality.

Are you a District Nurse with the Specialist Practice Qualification (SPQ) looking for the opportunity to develop your career? If you are, an exciting opportunity has arisen for an ambitious and enthusiastic Clinical Lead Nurse to join our District Nursing Service.


About the role

We are looking for a District Nurse who holds a Community Specialist Practitioner Qualification with significant Community Nursing experience to lead a Primary Care Network (PCN) aligned Locality team. The post holder will need to demonstrate a comprehensive specialist knowledge base and promote excellence in nursing practice in a dynamic healthcare environment. They will be responsible for the development and delivery of a 7‑day consistently high‑quality, safe and sustainable, cost‑efficient service operating 08:00 to 18:00.


Responsibilities

* Provide clinical leadership and line management for a Community Nursing team, working collaboratively with operational leads.
* Assess new and complex patients in their homes, plan and evaluate care, and deliver the Enhanced Health in Care Homes Framework in line with local objectives.
* Lead continuous quality improvement, support clinical governance, and manage caseload processes within the team.
* Help patients and carers achieve optimal independence using best practice and adhering to national and Trust guidelines.


Qualifications

Required:

* District Nurse qualification.
* Community Specialist Practitioner (SPQ) qualification.
* Significant experience in community nursing.
* Ability to travel independently, carrying equipment and car sharing as required.
