The postholder will join our existing team of 9 Pharmacists and 3 Pharmacy Technicians to support the 7 GP Practices of CWW PCN, as per rota scheduling to manage the caseloads of each Practice and delivering the defined requirements to meet the Primary Care Network DES and IIF.
This post will be managed by a Lead Practice, with sessions and clinical work taking place across the allocated Practices(s) weekly, as per rotas.
The postholder will be required to complete the CPPE Primary Care course if this has not already commenced.
Salary is Whole Time Equivalent (WTE) dependent upon PRIMARY CARE experience
Main duties of the job
The postholder is responsible for:
* Providing technical support to the Practices Clinicians of the 7 CWW PCN Practices
* Supporting the Practices to resolve day to day medicines issues and help patients to get the best from their medicines
* Supporting the Clinical Pharmacists dealing with complex queries around medication from patients, secondary care professionals, community pharmacists and other healthcare professionals
* Implementing medication changes with the support of the Clinical Pharmacists
* Completing audits relating to patient safety in the prescribing medication and presenting the information for sharing with the clinical team
* Ensuring required changes to medication are made safely and effectively
* Supporting delivery of national and local Primary Care vaccination programmes
* Supporting the PCN with health promotion
* Medicines reconciliation post discharge and other transfers of care, actioning changes where appropriate within competencies
* Undertaking reviews to ensure adherence to monitoring and shared care protocols
* Conducting phone consultations to offer patients medication advice and medication reviews
* Handling day to day prescription queries within area of own competence
* Reviewing nursing home prescription requests
* Carrying out housekeeping in patient records (e.g., dose optimisation and medicines synchronisation)
* Improving prescribing processes at the practice, delivering training and development to practice based prescribing clerks to support improved coordination and administration within the Practice
* Promoting electronic repeat dispensing and online ordering
* Assisting in advising clinicians of alternative medications when items are unavailable
* Assisting in practice response to complaints or significant events
* Liaising with other pharmacy stakeholders such as the Medicines Management Team, community pharmacy and hospital pharmacy teams
* Handling function specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and coordinating such information in accordance with the Data Protection Act, Caldecott Guidelines and the Confidentiality Code of Conduct
* Assisting with the development and review of medicines audits utilising the TPP GP practice clinical systems
* Ensuring all major MHRA drug alerts are acted upon within the practice within an appropriate timeframe and advising on any medication changes that occur as a result
* Promoting cost‑effective, safe, evidence‑based prescribing in accordance with national and local guidance
* Supporting students and trainees as required
* Participating in the administrative responsibilities of the practice team
* Ensuring the clinical computer system is maintained with accurate data entered in a timely manner
* Ensuring accurate completion of all necessary documentation associated with patient health care
* Reviewing and processing data using accurate read codes to enable easy and accurate retrieval for monitoring and audit purposes
* Collating, analysing and presenting clinical data and new guidance to the rest of PHCT for areas of responsibility
* Ensuring e‑learning modules are completed as per agreed timelines
* Implementing and adhering to the practices policies and procedures
* Adhering to practice procedures and policies to notify absences and request leave
* Conducting oneself in a professional, polite and friendly manner
* Maintaining a smart and professional image at all times ensuring uniform is kept clean, tidy and in good repair
* Working as an effective team member, supporting colleagues
