Team Early Intervention Worker Homelessness Prevention & Community Support

We’re looking for a passionate, proactive Early Intervention Worker to join our Homelessness Prevention team. This is a dynamic, community‑focused role designed to stop homelessness before it happens. What You’ll Do
* Work directly with individuals and families at risk of homelessness, offering specialist advice, targeted support, and practical solutions.
* Provide homelessness and housing advice, mediation and negotiation support.
* Manage a high and varied caseload, from low‑level needs to complex cases.
* Offer self‑help tools, signposting, and tailored support plans.
* Ensure homelessness duties are correctly applied.
* Connect clients with services such as health, debt management, substance use support, mental health services, community groups, and faith organisations.
This is holistic, person‑centred work that meets people where they are and helps them move forward safely.
Working Across the Community
You’ll be embedded in community schools, GP practices, hubs, outreach teams, and local organisations identifying risks early and acting quickly.
Collaboration will include:
* Housing and Homelessness Services
* Social care and public protection
* Healthcare providers
* Probation and criminal justice partners
* Community and voluntary sector organisations
You’ll also help shape prevention tools, deliver bespoke training, and keep partners informed of local and national policy changes. This role sits within the GMCA Live Well and Public Service Reform model, contributing to a truly multidisciplinary approach.
Ongoing Support & Monitoring
Maintain regular contact with clients and partner services, tracking progress and adjusting support as needed. Lead on multi‑agency coordination, ensuring every individual or family receives the right help at the right time to prevent homelessness.
Hours & Working Conditions
Normal hours of work are 37 hours per week based around the usual business working week. Dependent on service requirements, you may be required to work on a rota basis.
