The Home First service provides short-term, responsive, and flexible support aimed at helping individuals regain and maintain independence in their own homes.

The service plays a crucial role in preventing unnecessary hospital admissions and facilitating timely hospital discharges. Working in close collaboration with social care (KCC), community health teams, and acute hospitals, Home First supports patients during transition periods by offering enablement-focused care.

The service aligns with national integrated care strategies and is underpinned by a person-centred approach, empowering individuals to achieve outcomes that promote recovery, autonomy, and well-being in their home environments.

Main duties of the job

The Senior Assessor plays a pivotal role in delivering high-quality, person-centred care within the Home First service.

They provide advanced assessment, complex case management, and operational support, while also guiding and developing colleagues to maintain excellence across the team.

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Person specification

Qualifications

Essential criteria

1. Level 3 Diploma in Health & Social Care or equivalent
2. Working Towards Level 5 qualifications
3. Holding or willingness to work towards Qualified NVQ Assessor
4. GCSE English & Maths (Grade C/4 or above) or equivalent
5. Bachelor’s degree (BA/BSc), NVQ Level 6, or equivalent
6. Moving & Handling Train the Trainer qualification

Experience

Essential criteria

7. Background in Adult Social Care and or Health Care or previous first line supervisor/line management experience
8. Experience of supervising staff
9. Experience of working to contract specifications and delivery to required standards

Skills and Abilities

Essential criteria

10. Ability to undertake and document formal risk assessments
11. Ability to accurately assess client needs
12. Ability to communicate effectively with all levels of staff, people using the service and external contacts as required by the job
13. Ability to prioritise own workload in line with operational procedures and standards
14. Ability to monitor effectively others’ work performance
15. Proven administrative skills
16. Excellent negotiating and interpersonal skills
17. Ability to deliver services within budget and to contractual specification
18. Ability to promote the service in full
19. Ability to travel to meet the requirements of the service
20. Ability and commitment to support the Directorate’s Equality and Diversity Policy Statement, which is an integral part of the Directorate’s service delivery and relationship with the client to respect people as individuals regardless of age, ethnic origin, cultural values, disability, gender, sexual orientation or religion
21. This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements.

Knowledge

Essential criteria

22. Staff will be expected to work within national legislation and Corporate and Directorate policies and procedures relating to Health & Safety and those applicable to Health
23. Appreciation of Diplomas in the management of care and relevant assessment techniques.
24. Comprehensive awareness and understanding of the outcomes as set out in the Health & Social Care Act 2008.
25. Awareness of overall policies/procedures of department
26. Excellent understanding of enablement and how it fits the assessment process
27. Thorough awareness of legislative framework relating to the provision of enablement and domiciliary services
28. In depth knowledge of Safeguarding (Adult Protection) issues and need to promptly refer any concerns to Line Manager
29. Awareness of Data Protection and confidentiality issues
30. Awareness of and compliance with KCC equality and diversity policies, procedures and legislation

Personal Attributes

31. Full, valid UK driving licence, or a full, valid driving licence issued by an EU country (not exchanged from a non-EU country licence).
