A typical day as an Health Coach might include : Managing a caseload of clients in face to face clinics. supporting clients who want to quit smoking Supporting clients with Behaviour change to address various aspects of their health using SMART goal setting, taking into account the clients individual lifestyles/motivations and risk factors Attending regular team meetings and training sessions. Accurately recording client information on role specific databases Generate referrals Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources! Ensuring due consideration is given to our Safeguarding, Information Governance and Clinical Governance policies and procedures. To work with us in this role you will need to have some delivery experience of supporting clients to make healthy lifestyle choices.
