Summary:
The Community Health Worker role assists patients in accessing resources/services, supports disease self-management, and coordinates culturally-appropriate care and services.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
1. Education: High school diploma or GED preferred.
2. License/Certifications: If driving is required for the role, a valid driver's license, a clean driving record, and a personal vehicle for use are required.
3. Experience: Two years of experience working in a social service or healthcare setting or an equivalent educational setting preferred.
4. Additional Skills/Requirements (Required): Must be a resident of the community being served with good knowledge of the resources of their community.
5. Additional Skills/Requirements (Preferred): Basic computer skills preferred.
