Overview
The Forensic Intellectual and Neurodevelopmental Disabilities (FIND) Community Team is based at Jeanette Wallace House, 1 Edridge Road, Croydon, London, CR0 1FE (South London and Maudsley).
The service operates Monday to Friday (excluding bank holidays) from 09:00 to 17:00 and provides flexible, community‑based support to individuals across multiple London boroughs.
Key Responsibilities
* Deliver high‑quality, person‑centred care within patients’ homes and local communities, ensuring timely, compassionate and practical assistance.
* Provide safe transition support for inpatients discharged from forensic services, including specialist assessment, treatment and consultation.
* Oversee and manage a small caseload in partnership with the multi‑disciplinary team (MDT). Provide clinical assessment, treatment planning, implementation and monitoring of outcomes.
* Perform nursing procedures to the standards required by the Nursing and Midwifery Council and supervise/appraise junior staff and students.
* Facilitate user involvement in all aspects of service delivery, maintaining accurate and up‑to‑date records.
* Conduct risk assessments for clients and collaborate with the FIND Team Lead and MDT to manage high‑risk situations.
* Provide specialist nursing input for adults with learning disability (LD) and/or autism spectrum disorder (ASD) who also have co‑morbid mental illness or behaviours which challenge.
* Liaise with community mental health teams, forensic teams, courts, police liaison, probation, and other agencies to support clients across all South London and Maudsley boroughs.
* Maintain documented care plans and ensure clients have access to copies of their plans.
* Coordinate and facilitate specialist support for forensic and learning disability clients, including attendance at clinical, CPN, referral and professional meetings.
* Mentor allocated RMN/RNLD student nurses and joint practitioner students, participate in induction of new team members and provide teaching and supervision.
* Actively engage in clinical governance—identifying quality gaps, participating in audit programmes and contributing to the Care Improvement System.
* Provide oral evidence to Magistrates’ courts and participate in the assessment and screening of mentally disordered offenders.
* Support the safety and well‑being of service users and staff, maintaining accurate incident and near‑miss reporting in line with the Trust’s policies.
* Maintain professional development, including participation in research, clinical audits and ongoing training.
* Ensure confidentiality and compliance with the South London and Maudsley Hospital NHS Foundation Trust policies and national regulations.
* Facilitate access to mainstream health services, using tools such as Hospital Passports and individual health action plans where appropriate.
* Promote the healthcare needs of people with a learning disability and/or ASD in line with national standards and strategic priorities.
