Occupational Therapist Assistants (2 positions)
Hackney Council is looking for two Occupational Therapist Assistants to join the new Reablement Team within the Integrated Discharge Service. The role supports residents discharged from hospital, helping them regain independence and confidence in their homes or Reablement Assessment Flats.
About the role
* Carry out strengths‑based assessments, equipment reviews and follow‑up visits under a qualified OT.
* Recommend and arrange specialist equipment, minor home adaptations and mobility support (e.g., Blue Badges, Tech‑Enabled Living).
* Provide clear advice or signposting to other services; promote resident wellbeing and identify risks.
* Maintain accurate records on the council’s IT systems, prepare reports and manage your caseload with strong time management.
* Actively support residents, carers, families and partner agencies to ensure continuous, person‑centred care.
Key requirements
* Experience or accredited Trusted Assessor qualification (advantage but not essential).
* Knowledge of the Care Act and related legislation; strong working knowledge of specialist equipment, home adaptations and assistive technology.
* Ability to make sound clinical decisions, manage caseloads and record work clearly.
* Excellent communication skills; able to build positive relationships across diverse organisations.
* Commitment to strength‑based, person‑centred practice and continuous professional development.
* An enhanced DBS check is required.
