Position Overview
As the Multidisciplinary Team (MDT) Coordinator you will be part of the Integrated Care Team providing non‑clinical support to the Clinical Manager, Community Matrons and District Nursing Teams. You will coordinate a team of professionals from multiple disciplines to deliver proactive, person‑centred care for patients with complex needs and frailty.
Key Responsibilities
* Manage a caseload of frail or complex patients and create MDTs around each patient.
* Lead multidisciplinary discussions to set and implement individual care plans.
* Act as the single point of contact for health and social care professionals to access the Frailty Pathway.
* Maintain an up‑to‑date knowledge of local services, support networks and community resources.
Qualifications & Experience
* Relevant health or social care experience with complex care planning.
* Strong collaboration and facilitation skills across multiple disciplines.
* Knowledge of local frailty services and community pathways.
