Children with Disabilities Social Worker – Medway Council
Medway Council is recruiting a qualified Social Worker to join its Children and Young People with Disabilities (CWD) Team, delivering high-quality, relationship-based statutory social work to disabled children, young people and their families.
You will manage a varied caseload, completing analytical assessments, SMART care planning and reviews, ensuring children and young people are safeguarded and supported to achieve meaningful outcomes and successful transitions into adulthood. Practice is underpinned by a strengths-based and Signs of Safety approach, with a strong emphasis on reflective supervision and professional development.
Key Responsibilities:

Manage a caseload of children and young people with disabilities within statutory frameworks
*
Complete robust assessments, care plans and reviews with clear outcomes
*
Build strong, trusting relationships with children, young people and families, ensuring their wishes and lived experiences are central to practice
*
Prepare high-quality reports for Child Protection Conferences, CIN meetings, LAC reviews and court proceedings
*
Work collaboratively with multi-agency partners, including the MASH
*
Maintain accurate case records in line with GDPR and Medway Council systems
*
Participate in a duty rota and regular reflective supervision
*
Maintain CPD in line with Social Work England requirements
What Medway Is Looking For:

Qualified Social Worker (Social Work England registered)

Experience working with children with disabilities and/or complex needs

Confident in strengths-based, relationship-focused practice
