Who You Are We are looking for committed individuals with a caring nature to join The Community Resource Team (CRT) Homecare in Cardiff. You should possess good observation and recording skills, and be flexible and focused to work in a dynamic, fast-changing environment. The ability to speak Welsh or other Community Languages is advantageous. Candidates must have a full clean driving licence and use of a car. Commitment to achieving the relevant HSc (previously NVQ/QCF) level is required. What the Job Involves As a member of the CRT Homecare, you will support citizens in Cardiff by preventing unnecessary hospital admissions and facilitating hospital discharges. You’ll collaborate with health professionals to help clients achieve their reablement and wellbeing goals. Duties are guided by a service delivery plan and allocated via a mobile device. Multiple positions are available, with various rotas including weekdays, evenings, and weekends.
