We deliver high-quality,excellent palliative care for patients across Nottinghamshire for those livingwith a life-limiting condition. We care for patients, families and theirfriends, both before and after death.

Most people are cared for in their own home & our goalis to help people make the most of their lives, in ways that are important toTHEM. This care may sometimes include coming into the Hospice as part of ourwellbeing and reablement offer.

We are currently looking to recruit a dynamic andexperienced Registered Nurse to lead and develop our growing Hospice In YourHome offer and operationally manage the team.

Our Hospice In Your Home Team provides planned 1:1 care and a roaming service to people in their own homes across Nottinghamshire. The successful postholder will work shifts across the week, including weekend and bank holidays, on a planned rota basis and across the region based on patient need and referral criteria.


Main duties of the job

You will be required to work 36 hours per week worked overfour 9-hour shifts.

Shifts will be worked across a 7 day / 3-shift roster.Monday to Sunday (early) 7.00am-4.00pm, (late) 4.00pm-1.00am and (Night)10.00pm-7.00am

We provide a range of palliative care services for peoplein Nottinghamshire, and we are currently looking to recruit a dynamic andexperienced HIYH Registered Nurse Lead to develop our growing Hospice In YourHome offer and operationally manage the Care Team.

You must have substantial experience of leading teamspreferably gained within a relevant healthcare setting. You will possessoperational management experience and leadership skills. Support will be givento the postholder to develop their career ambitions. Essential to this role is an understanding ofmulti-disciplinary working, within healthcare, experience of managing teams anda passion for hospice care. In addition,you must have excellent communication, planning and networking skills, theability to prioritise a busy workload and proficiency in the use of MicrosoftOffice and clinical systems. Experience of SystmOne is an advantage. Servicedevelopment and transformation will be key aspects of the role. Flexibleworking on occasion may be required to meet the needs of patients.

You must be a Registered Adult Nurse with post registrationexperience. You must also possess a fullUK driving license and have access to a vehicle.

Job responsibilities

As part of the Hospice in Your Home Team the post holderwill work as a HIYH Registered Nurse Lead to support palliative and end of lifecare patients within their own home. The HIYH Team provides Hospice at Home andHospice Night Support Services over 24 hours, 7 days per week, including BankHolidays and shifts are allocated on a roster basis. The key responsibilitiesand duties are to:

* Provide caseload management and lead the delivery ofhigh-quality palliative and end of life care that delivers on CQC and qualitystandards for the Hospice In Your Home Services.
* Promote the services and patient outcomes achieved byNottinghamshire Hospice, liaising with Healthcare Professionals on clinicalissues in relation to the provision of care.
* Provide line management for allocated HIYH RegisteredNurses (contracted) and Registered Nurses (Bank) and support them with the linemanagement of HIYH Palliative Care Assistants (contracted) and Health CareAssistants (Bank) within the Hospice in Your Home teams.
* Act as the nominated Shift Leader
* Undertake Clinical On-Call on a roster basis
