We are looking for a motivated Advanced Nurse Practitioner to join our friendly and hardworking team. Flexible sessions available.
We are a forward-thinking, two partner training practice with a growing list size of 9,585 patients. We also have a small dispending branch surgery in Whitfield.
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
Assess, diagnose and offer treatment to patients with minor illness, supporting and advising the wider MDT on best practice.
Proactively identify, diagnose, and manage treatment plans for patients a risk of developing a long-term condition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.
As a training and teaching practice, there is a strong culture of learning, mentorship and professional development. The wider clinical team includes experienced GPs, ANPs, Nurse Practitioners, Pharmacy Technicians as well as a dedicated administrative team, all working together to maintain a positive and patient-centered atmosphere.
Demonstrate safe, competent clinical decision-making and expert care within scope of practice, including assessment and diagnostic skills, for patients within the general practice.
The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting delivery of policy and procedures.
The post holder will need to be able to interpret, comment and action pathology, microbiology and radiology results.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
