The Support Planner role will join a small and friendly team based in Nottingham City. You will need to travel for this role but will either receive a travel pass or mileage paid and parking passes. Your office based work will then be hybrid office and home based. The RSAP Team support people in various locations around central Nottingham. We support individuals with multiple complex needs in their own tenancies which include self-contained flats and shared housing. Our team is made up of Support Workers and Support Planners who aim to support tenants toward attaining permanent accommodation. We provide support in areas including housing, money management, substance misuse and liaise with external agencies who specialise in these areas. Duties & Responsibilities To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs. You will have caseloads with Tenants in properties across the city (mileage system for car drivers and public transport paid). Prepare Support plans. Sign post Tenants onto appropriate services for substance misuse management & Assist service users with housing applications and move on options. Assist service users with basic budgeting and life skills in order to support them to move on to live independently. To liaise appropriately with Key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users. Duties and Responsibilities To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs. Be responsible for assessing and evaluating the needs of Service Users. To ensure that tasks identified in support plans are communicated to Support Workers and that these tasks are clear, appropriate and agreed with the Service User. To enable Service Users to understand the requirements of independent living including budgeting, maintenance and use of leisure time. To liaise appropriately with Key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users.
