Please note there are two fixed term vacancies of 29.25hrs per week - 1 in Shrewsbury and 1 in Oswestry.

We are seeking a compassionate, skilled, and motivated Band 6 Mental Health Nurse to join our specialist Dementia Home Treatment Team within the Shropshire, Telford and Wrekin locality.

This is an exciting opportunity to be part of a dynamic and supportive multidisciplinary team delivering high-quality care to improve the quality of life for people living with dementia, their families, carers and loved ones.

The role involves working closely with colleagues across the multidisciplinary team and liaising with GPs, social care, and voluntary sector partners to ensure continuity and quality of care. You will maintain accurate and timely clinical records in line with Trust policies, in addition you will contribute to service development, quality improvement initiatives, and provide professional advice, support, and supervision to colleagues where appropriate.

The role requires significant experience and knowledge of dementia, managing a complex caseload, excellent communication skills, team working and the ability to work under pressure to respond to high service demand.

The role is community based and therefore it is essential that you have a full UK driving licence and the use of a car.


Main duties of the job

* Take responsibility, for the management ofa complex caseload, whichincludes, specialist assessment (withinservice specification), care planning in conjunction with service user and carer, evaluation of individualtreatmentplans(CareProgrammeApproachandSingleAssessmentProcess).
* Responsible for participating in the setting ofquality standards, including the auditing, monitoring and reviewingof practice in line withcurrent clinical guidance practice and policy.
* Assess patient's individual care needs, develop, implement and evaluate programmes ofcare.
* Deliverarangeof interventionsinaccordancewiththeagreedplanofcareandPathway guidanceincludingpharmacologicalandpsychologicallyinformedinterventions.
* Complete and contribute, where appropriate to robust riskassessments and riskmanagement plans. Deliverand promote positiverisk taking, where appropriate, to improvequalityoflifeandmaximiseoccupationalperformance/independence.
* Maintain an effectivereporting system by observing and reportingverballyand in writing on patient conditions.
* Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
