About Sustain Clackmannanshire
Aberlour Child Care Trust has recently launched a new family outreach service, Aberlour Sustain Clackmannanshire.This service works alongside families, communities and partners to offer caring and responsive support to youngpeople and families. Aberlour Sustain Clackmannanshire delivers the support families need, when they need it,every day of the year. The aim of the service is to ensure that children and young people can continue toflourish at home, at school and in their local community.
Working with Social Work Services, this innovative and creative new service focuses on supporting young people aged 0– 18 years, who may be on the ‘edge of care’ so that they remain living with their family in their localcommunities.
Our Service works with young people and their families to enhance relationships, build on existing strengths andinterests, help build confidence and encourage new community connections. Our aim is to help all families feelsafe, confident, capable, and included. Our approach has been developed through consultation with families andpartners.
What we are looking for....
We are looking for a Lead Practitioner who is passionate about supporting young people, families and communities, whois flexible and shares our vision that collaborative working and relationship based early intervention is thebest option for young people and families who are feeling overwhelmed or excluded.
Not afraid to test new ways of working, you will bring a fresh, caring, compassionate and collaborative approachwhich responds empathically to the needs of young people and their families. The work will include earlymornings, evenings and weekends. You will have experience of direct practice within this field of work,including child/adult protection issues as well as having experience of working with children and youngpeople.
You will work within a variety of locations in the Clackmannanshire area, including the office base in Alloa, familyhomes, schools and community settings.

About Aberlour Options Fife - Respite and Outreach
Options Fife Respite and Outreach provides a range of flexible support through outreach and overnight short breaks atweekends, evenings and school holidays to children and young people with complex learning disabilities andautism. We offer a high-quality child centred service providing positive and enabling experiences for the youngpeople, supporting them through the challenges and complexities that enable them to work towards theirindividual outcomes and personal goals. We work closely with families and other professionals in providingsupport tailored around the child’s plan.
What we are looking for....
We are looking to recruit two Young People’s Worker Nights, working four nights on (10 hours per night) every secondweek. You will play a fundamental role in ensuring that the young people you work with have the best possibleexperience in working towards their objectives in line with their care plan, supporting children to progresswithin their individual outcomes. Whilst not without its challenges and demands, this is a highly rewardingrole, where the work we do has a demonstrable impact on each young person's progress. Using a person-centredapproach, you will provide enabling support ensuring the highest level of physical, personal, and emotionalcare. We support young people who have epilepsy, need personal care and need gastrostomy feeding. Aberlour, inconjunction with NHS, provides all staff with training.
We are looking for candidates who have a passion for working with young people with disabilities. It is desired butnot essential that candidates hold a relevant qualification at an SCQF level 7 or above. About the service
Our Silverton Short Breaks Service offers support to children and young people from the ages 5yrs – 18yrs who havedisabilities. We work to reduce the impact of the disability on the child's day to day life by providingsupport for them to participate, make choices, enjoy new experiences, make friendships and be part of theirlocal community. At the same time parents and carers can have a much-needed break knowing their child is in asafe, fun and enriching environment.
How you'll help to create brighter futures
* Provide lead case/care planning management, including assessing, reviewing and managingrisk.
* Co-ordinating responsibilities within the service, with multi agency and producing familyinformation, including analysis and written reports.
* Attendance at professional meetings includingreviews, court work etc.
* Making sure our young people are safe and cared for when they are away fromhome.
* Assisting in all aspects of their daily lives and supporting their health andhappiness.
* Supporting and participating with keyworkers to maintain an up-to-date Plan for youngpeople.

Are you resilient and have a positive approach to change and challenge? We're looking for someone with a ‘cando' attitude to join our team. Ideally you'll have:
* Demonstrable experience of working in a care setting and developing positive relationships through trust.
* Application of a range of techniques to co-ordinate and communicate with service users and their families.
* Demonstrate a solution focused approach to address problems, individually and in a team setting.
* Demonstrate emotional resilience to respond to the challenging nature of the role.
* Possess a Level 3 Diploma in Residential Childcare (or recognised equivalent) or if you do not possess this you must be prepared to complete this within 2 years of commencing employment with us, full support will be provided.
