The position is full-time, working across any of the six days from Monday to Saturday, at one of three sites: City Health Campus, Sandwell Health Campus, or MMUH. You will work within a multidisciplinary team, maintaining regular contact with nursing, medical, and administrative staff.

1. Assessing, planning, implementing, and evaluating individual care programs based on an agreed model and considering individual needs, promoting health education.
2. Managing a defined group of patients from admission to discharge.
