Job Title: Specialist Care Support Worker - Fixed Term for a period of 12 months x 2
Location of post: Trem Y Mor Day Service Scarlet Avenue Port Talbot SA12 7PH
Your workstyle in this role will require you to work from a fixed workplace.
The Role
We are seeking Specialist Support Workers to join our dedicated team at Trem Y Mor Day Services. The service operates Monday to Friday and provides a structured programme of activities for individuals with complex learning disabilities. Trem Y Mor supports individuals within a caring and inclusive environment, enabling them to achieve their personal outcomes through participation in a wide range of activities, both on site and within the local community. The service actively promotes independence, health, and wellbeing. This role requires skilled and compassionate staff who are confident in supporting individuals with complex and additional needs and who are able to work in line with the care and support plan.
As a Specialist Support Worker, you will support individuals to:
1. Feel safe, respected and valued
2. Maintain dignity, choice and independence
3. Access meaningful activities and short breaks
4. Receive consistent, person‑centred care
Key Responsibilities
5. Provide high‑quality, individualised support in line with care plans and risk assessments
6. Support individuals with complex needs,
7. Implement Positive Behaviour Support strategies consistently
8. Assist with personal care, medication (where trained), nutrition and wellbeing
9. Maintain accurate and timely records (care notes, daily logs, incident reports)
10. Work collaboratively with families, health professionals and social workers
11. Promote safeguarding and follow organisational policies at all times.
Essential Criteria
12. Experience supporting individuals with complex needs in a care setting
13. A calm, patient and professional approach
14. Good written and verbal communication skills
15. Understanding of safeguarding responsibilities
Desirable (but not essential)
16. Experience with PBS, autism, complex learning disabilities
17. Registration Social Care Wales (immediate application upon commencement)
18. NVQ/QCF Level 2 Health & Social Care (or willingness to work towards)
