Overview

The PCN Manager will need to work closely with practice teams, healthcare professionals, and external partners across the PCN which could involve working across other PCN sites. The PCN Manager plays a vital operational and coordination role within the PCN in North Hammersmith & Fulham, supporting the delivery of high-quality, integrated, and population-focused primary care. The role ensures the effective day-to-day functioning of the PCN, driving the implementation of key priorities from the NHS Long Term Plan, and enabling transformation at neighborhood level.

Role Responsibilities

1. 1. PCN Coordination & Delivery
Oversee the day-to-day coordination of North H&F PCN, ensuring delivery of national PCN service specifications and ICB plans. Manage PCN-level projects including Enhanced Access, anticipatory care, and digital transformation initiatives. Maintain effective communication across practices, supporting a consistent approach to service delivery and patient engagement.
2. 2. Programme & Performance Management
Support delivery of PCN and ICB transformation projects and programmes, ensuring KPIs, reporting, and timelines are met. Prepare performance reports for the Non-Clinical Director and Clinical Director, highlighting delivery risks and progress against objectives. Track impact and outcomes of local initiatives, using data to inform planning and improvements.
3. 3. Workforce & ARRS Management
Coordinate recruitment, induction, supervision and retention of the Additional Roles Reimbursement Scheme (ARRS) workforce. Maintain workforce plans aligned with patient need and ICB funding allocations. Work with the Training Hub to ensure PCN staff have access to development opportunities and structured support.
4. 4. Financial & Contract Management
Support the Non-Clinical Director and Clinical Director in managing the PCN budget, ensuring accurate financial planning, claims, and use of ARRS funding. Oversee contracts and service agreements related to PCN projects and providers. Ensure compliance with ICB policies and governance arrangements.
5. 5. Digital & Operational Support
Lead on the operational management of contracts and workstreams on behalf of the PCN. Support practices in implementing digital solutions, such as online triage, shared records, and patient access systems. Facilitate operational improvements that reduce duplication, streamline workflows, and enhance care delivery. Liaise with IT and digital leads across NWL to ensure technical alignment and problem resolution.
6. 6. Stakeholder Engagement & Communications
Serve as the main point of contact for internal and external stakeholders relating to PCN operations. Organise and support PCN meetings, including senior managerial meetings, staff forums, and public engagement sessions. Promote a positive, collaborative, and transparent working culture across the PCN.
7. 7. Mandatory and Statutory Training
To participate in LCW performance review and appraisal process. To comply with mandatory and statutory training requirements. To agree sector specific ongoing personal development/training needs required for the role.

LCW is the employing organisation for roles within North H&F PCN and holds the NWL Standard contract for the same PCN. LCW runs the Partnership for Health GP Practice within the PCN (in partnership with Imperial NHS Trust).

Job responsibilities (expanded)

The PCN serves a well-defined geographical area that is roughly contiguous with the W12 Postcode. This is an area characterized by social housing and socio-economic deprivation, with 80% of residents falling into the most deprived two IMD quintiles and about 19,000 people being within the core 20 cohort. The needs of this population are met by nine GP practices, which have a combined list size of about 58,000 and include:

1. Canberra Old Oak Surgery
2. Dr Canisius & Dr Hasan (Parkview Centre for Health & Wellbeing)
3. Dr RK Kukar (Parkview Centre for Health & Wellbeing)
4. Dr Uppal & Partners (Parkview Centre for Health & Wellbeing)
5. Hammersmith & Fulham Centres for Health
6. Shepherds Bush Medical Centre
7. The Medical Centre
8. The New Surgery
9. Westway Surgery

These practices are arranged in a hub and spoke configuration, with four based at a purpose-built Centre for Health and Wellbeing in the centre of the PCN geography, two to the north in Old Oak, and three to the south along Uxbridge Road. The PCN can be walked from north to south in about 15 minutes.

Qualifications, Experience & Skills

Essential

* Degree or equivalent working experience.
* Experience in NHS primary care or service coordination.
* Experience of operational management and service improvement work.
* Financial acumen and experience with budget monitoring and reporting.
* Strong organisational and planning skills with the ability to manage competing priorities.
* Excellent communication and stakeholder engagement skills.
* Strong IT skills including MS Office and practice systems.
* Flexible and adaptable to change to meet the needs of the role.

Knowledge

* Understanding of PCNs, ARRS workforce, and NHS Long Term Plan objectives.
* Continuing professional development and industry learning
