Responsible to: Senior Staff Tabernacle Medical Centre Location: Working across 7 General Practices / based in Tabernacle Surgery Accountable to: Neath Cluster Network Hours of work: To be determined by the 37.5 hrs per week Salary: To be determined by the host Practice and Cluster [ note : the post will be eligible for reimbursement through the Network Contract and is based on indicative AfC Band 7] - £44,398 - £50,807 depending on experience. Duration: Until March end - 2026 We are seeking to recruit a Clinician to work within The Neath Cluster Collaborative, working alone but as part of an expanding multi-faceted range of cluster funded programmes. Neath Cluster is one of the eight Clusters in Swansea Bay University Health Board, and comprises eight General Practices delivering services from eight sites. The Cluster serves a population of about 57,000 patients. The successful candidate will join the network and will continue to provide a high standard of clinical care by creating a service that effectively and efficiently meet the needs of our patients. Serving patients, across Neath Cluster Network we are seeking an individual who strives to develop collaborative working across the 8 member practices, aiming to support and strengthen the delivery of primary care services in line with the Primary Care Plan and Local Cluster Collaborative going forward. Working alongside others the post we align to a range of Cluster initiative including GPs, a Clinical Pharmacy Team, and a Wellbeing team comprising of: Social Prescribers and link workers, as well as access to a range of community based clinics such as Audiology, Physiotherapy working across the Cluster. Offering flexible working patterns and ongoing supervision, support and training we ensure our staff are supported within a professional working environment and maintain an effective work life balance. The successful applicant will be responsible for triaging patients as well as seeing patients both within Practice and at home, with a wide range of minor illness, urgent and same day requests for treatment. They will assess and triage patients providing treatment or referring onto other members of cluster and non cluster teams. The post holder will attend home visits to include residential and nursing homes, where they will liaise with staff, family and other stakeholders in the production of care plans to be shared with other providers including the Out of Hours (OOH) service. The post holder will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. Primary care experience would be an advantage however is not essential. Flexibility, enthusiasm and good clinical knowledge are essential. Successful candidates will be supported in terms of clinical learning and professional development. Key duties and responsibilities 1. assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the NCN team 2. advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services. 3. be able to: a. perform specialist health checks and reviews b. perform and interpret ECGs; alongside other results as appropriate c. perform investigatory procedures as required d. undertake the collection of pathological specimens including intravenous blood samples, swabs, etc. e. perform investigatory procedures needed by patients and those requested by GPs and the wider NCN 4. support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing or other such support with a range of Chronic Conditions) 5. provide an alternative model to urgent and same day home visits for the NCN and undertake clinical audits 6. communicate at all levels across NCN and other organizations, ensuring effective, patient-centred service 7. communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required 8. collaborate with other members of the NCN including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care 9. maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice 10. prescribe (or liaise with GP to) issue and review medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice 11. work with patients in order to support compliance with and adherence to prescribed treatments 12. provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions 13. assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs 14. clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly 15. prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care 16. diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan 17. support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care 18. Support GPs to manage daily house call commitments and provide opportunity for advanced chronic condition reviews. Quality Requirements 19. enhance own performance through continuous professional development, keep up to date with current evidence-based practice, and impart own knowledge and skills to NCN colleagues to meet the needs of the service 20 recognise and work within own competence and professional code of conduct as regulated by the HCPC and College of Paramedics / Relevant nursing / health degree with NMC 21 assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation 22 participate in research and utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required 23 in partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate Administrative requirements 24 produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures 25 be aware of data protection (GDPR) and confidentiality issues particularly within a NCN 26 use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information 27 review and process data using accurate read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes. Supervision The post holder will have access to appropriate clinical supervision and an appropriate named individual in the NCN to provide general advice and support on a day to day basis.
