Advanced Clinical Pharmacist - Independent Prescriber
The post holder will be an independent prescriber and an experiencedhealthcare practitioner with experience in acute and urgent care. They willhave completed an MSc in Advanced Practice, or MSC Pharmacy, and ideally havecompleted the Primary Care Pharmacy Education Pathway with CPPE. The post holder must be able to evidence aclinical portfolio and relevant competencies in line with the HEE nationalframework for advanced clinical practice. Advanced clinical practitioners areexpected to work across the four pillars of advanced practice; clinical,leadership, research and education.
The post holder will practice as an Advanced ClinicalPractitioner/Pharmacist across various services, including but not limited toABC Remote Services (uCATS, Enhanced Access, Additional Primary Care)
Main duties of the job
The post holder will autonomously undertake assessment andtreatment of both acute and chronic, differentiated and undifferentiated,patient presentations.
The post holder will determine a clinical diagnosis,treatment plan and appropriate discharge, or refer, as indicated whilstmaintaining accurate patient records and ensuring patient safety. They will bea source of clinical expertise to less experienced staff, enhancing skills andknowledge, and work in partnership with a multidisciplinary team.
The post holder will ensure a high-quality patient centredservice is delivered in remote settings across all clinics.

Primary duties and areas of responsibility
Clinical responsibilities
1. Toindependently undertake specialised clinical assessments, diagnose, treat anddischarge, or appropriately refer, patients with a wide range of acute andchronic illnesses.
2. Toensure patients are fully involved in the design and implementation of theirtreatment plans.
3. Toensure effective coordination and liaison with multidisciplinary teams acrossprimary and secondary care, statutory and voluntary disciplines/groups toprovide a seamless service to patients and their carers and prevent hospitaladmission where urgent care is required.
4. Torefer appropriately to clinical leads/supervising GP where a second opinion isrequired.
5. Toadapt styles of communication appropriately to promote empowerment,self-management, and health education for patients.
6. Toattend clinical case conferences, review and clinical meetings as required.
7. Tobe a source of clinical expertise to less experienced staff within the team.
8. Todelegate duties/tasks to other members of staff where appropriate.
9. Providespecialist advice relating to minor illness and/or injury to local healthcareteams, patients, relative, and carers.
10. Toidentify actual or potential clinical risks and take appropriate action using,when required, the appropriate reporting structure.
Leadership / Service Delivery Responsibilities
1. Toensure that patient records and documentation are maintained in line withrecognised standards and ABC policies and use of the electronic patient recordssystem. This includes
Documenting full and accurate record ofassessments, investigation, procedures, treatment, and aftercare.
Supporting colleagues to maintain good andcomplete records.
Working closely with the administration and/oroperations team, ensuring all records pertinent to patients are filed asappropriate.
Contributing to the read-coding of patient data.

3. Toparticipate in audits and use the results, current research and governmentguidelines, to make recommendations for changes to service delivery andclinical practice, and to share knowledge with other healthcare professionals.
4. Tocontribute to the maintenance and improvement of communication networks withall disciplines to ensure continuity of care.
5. Tocollect data and provide service information to the Service Manager asrequired.
6. Tocontribute to the development and establishment of systems to gather feedbackfrom patients.
7. Toensure that service relevant Managers and Clinical Leads are kept informed onservice issues and potential risks.
8. Towork with the team in delivering and promoting the needs of the service.
9. Towork with the Service Manager and Clinical Lead to meet the clinical governanceneeds of the service; including investigation of incidents (including seriousincidents) and complaints, where required, sharing learning that arises frominvestigations.
10. Toprovide clinical supervision / mentorship (where appropriately trained) tojunior practitioners.
11. To remainupdated on the use of all equipment used and to report any technical problemsor issues with that equipment.
12. Toparticipate in clinical team meetings to discuss day-to-day operation issues.
