Job overview

We are seeking an experienced and motivated professional to join us as Intensive Home Treatment Team Leader in Bradford, based at Lynfield Mount Hospital.

Working closely with the Clinical Manager, you will play a key leadership role in ensuring high-quality, patient-centred care, supporting individuals with complex mental health needs in the community.

This is an exciting opportunity to make a real difference by driving service excellence, fostering strong partnerships, and supporting a multidisciplinary team to deliver outstanding outcomes.

Main duties of the job

The post holder will have managerial responsibility for a team of clinicians and support workers and may have responsibility for an environmental area within the Adult Mental Health Acute Care Service.

You will also have continuing responsibility for a comprehensive high quality mental health service within a designated area of the Adult Mental Health Acute Care Service.

Right to Work in the UK

Qualifications

Essential criteria

  • Registered Mental Nursing Certificate (RMN) Current Registration on part 3 or part 13 of NMC register. Or Occupational Therapist registration HPC Or Social Workers registered GSCG with the Assessed and Supported Year in Employment
  • Evidence of continuing professional development.
  • Desirable criteria

  • Masters level qualification

  • Advanced Nursing Qualification
  • Experience

    Essential criteria

  • Experience of working at senior nurse level (band 6 or equivalent) for at least 18 months.
  • Clinical experience including experience of risk assessment, assessing patients and creating/implementing and evaluating care plans etc.
  • Experience of working with people with long term Mental Health Needs.
  • Experience in taking charge of shifts and acting as Duty Nurse.
  • Experience of demonstrating clinical leadership.
  • Experience of staff supervision and supporting students.
  • Experience of providing teaching and education to staff.

  • Involvement in innovative practice
  • Experience in team leadership and management
  • Experience in budgetary control
  • Knowledge

    Essential criteria

  • Assessment, planning, implementation and evaluation of individualised care plans
  • To be able to ensure the compliance of the unit with the Mental Health Act.
  • Knowledge of current professional issues
  • Knowledge of relevant professional Mental Health legislation/guidance and Community Care Legislation and Guidance
  • Knowledge of quality issues within mental health environments
  • Understanding of the strategic implications of operational issues and how this fits with the Trust’s aims.
  • Aware of racial, culture and gender issues
