The Palliative & Complex Care Team (PACCT) provides short-break respite care for children with life-threatening illnesses. The team is looking to recruit a Family Health Support Worker / Short Break Respite Worker working 22.5 hours per week.
In this role, you will participate in planned care of children, observe and contribute to the assessment of the child, recognize special needs or problems, and identify ongoing support as required.
The respite care and support are provided flexibly to meet the needs of the child and family. Care is provided over 5 days a week, from 9 am to 5 pm. You will be expected to work independently at times and will need to drive to multiple locations daily.
The caseload covers the geographical area of Stoke-on-Trent and North Staffordshire across various community settings.
We are seeking candidates with experience working in a care environment with children. Responsibilities include:
1. Visiting children and assessing care needs within the home to gather baseline information for care planning, developed with the child's family under the guidance of a qualified children's nurse or named nurse.
2. Supporting families by working independently after completing necessary competencies and assessments, with support from registered nurses.
3. Working flexibly to meet the needs of children and families, taking responsibility for healthcare and play needs in the absence of parents/carers, including medication administration (potentially including rescue medication).
4. Assessing and evaluating planned care programs in collaboration with the PACC Team, and providing regular feedback on the child's progress or changes.
5. Using essential equipment such as suction machines or hoists following appropriate training.
