Overview
Join the City of York Council as a full‑time Social Care Worker in the Adult Social Care Community Team (ASCCT). The role is open until 30/06/2026 and is hybrid, requiring a minimum of 2 days per week in the West offices.

Responsibilities
* Carry out Care Act 2014 assessments, mental capacity assessments and Best Interests meetings.
* Work collaboratively in multidisciplinary teams with health professionals, safeguarding, support planning and care management of adults with long‑term physical and mental health needs.
* Utilise community services locally where possible and provide holistic support to service users.
* Cover duty work on a rotational basis (previous duty experience is beneficial).
Qualifications & Experience
* Experience completing assessments and a strong knowledge of the Care Act 2014.
* Ability to deliver care in a high‑quality, client‑focused environment.
* Excellent communication skills; able to converse with the public at CEFR Level C1 in spoken English.
* Eligibility for an enhanced Criminal Record check via DBS.
