Asa Pharmacy Technician you will support our GP practices and work closely withour team of Clinical Pharmacists. The successful candidate will share ourvalues and have the energy, skills and enthusiasm to contribute to shaping howwe support our patients. You will be required to act within your professionalboundaries, supporting and working alongside a multi-disciplinary team withingeneral practice and a Primary Care Network.
Thepurpose of the role is to lead improvements to maximise safe, cost effectivebest practice in prescribing to improve the quality of patient care. The postholder will help patients to get the best from their medicines by switchingmedications to agreed and approved protocols, improving repeat prescribingprocesses in General Practice, promoting digitalisation, minimising clinicalrisk, and aiming to reduce wasted medicines. In addition, you will beresponsible for encouraging the development of better understanding of theprinciples of medicines optimisation throughout the practice teams andpromoting good practice in line with therapeutic developments. This willinvolve assisting the PCN in achieving national requirements, implementation ofNICE guidelines, and utilisation of medicines optimisation initiatives.
Main duties of the job
* Undertake patient facing and supporting roles to ensure effective medicines use
* Carry out medicines optimisation tasks including effective medicine administration, reconciliation, and actioning hospital-initiated medication changes.
* Utilise skills to work with patients to ensure they use their medicines effectively
* Support medication reviews and reconciliation for care home patients. And synchronise medicines for patient transfers between care settings and linking with local pharmacists
* Provide expertise to address the public health and social care needs of patients, including lifestyle advice and service information
* Take a central role in the use of shared care protocols and liaising with specialist pharmacists for more complex patients
* Support initiatives for antimicrobial stewardship
* Assist in the delivery of medicines optimisation, management incentive schemes and patient safety audits;
* Support the Pharmacy team with Structured Medication Reviews
* Work with the PCN multi-disciplinary team to ensure efficient medicines optimisation, including implementing efficient ordering and return processes, and reducing waste
* Provide leadership for medicines optimisation systems, support practices with a range of services by encouraging and implementing Electronic Prescriptions, safe repeat prescribing systems, and timely monitoring and management of high-risk medicines
* Provide training and support on the legal, safe and secure handling of medicines

* Undertake theCPPE Primary Care Pharmacy Education Pathway (PCPEP), as per the requirementsof the PCN Additional Roles Reimbursement Scheme
* Takeresponsibility for own development, learning and performance and keep up todate with their own requirements for continuing professional development (CPD)
Clinicalresponsibilities

* Carry outmedicines optimisation tasks including effective medicine administration (e.g.,checking inhaler technique), medicines reconciliation, and actioninghospital-initiated medication changes. * Support, asdetermined by the PCN, medication reviews and medicines reconciliation for newcare home patients and synchronising medicines for patient transfers betweencare settings and linking with local community pharmacists.

* Take a centralrole in the clinical aspects of shared care protocols and liaising withspecialist pharmacists for more complex patients;

* Developrelationships with other pharmacy technicians, pharmacists and members of themultidisciplinary team to support integration of the pharmacy team acrosshealth and social care including primary care, community pharmacy, secondarycare, and mental health teams.
Supervision
* The postholderwill have access to appropriate clinical supervision and an appropriate namedindividual in the PCN to provide general advice and support.
